To better serve our Pharmaceutical clients, Particle Technology Labs holds a DEA License which allows us to test Schedule II, III, IV and V drugs.

All DEA drug samples must be accompanied by a copy of your current DEA License.  Your project cannot be scheduled until a copy of the license has been received.

DEA Schedule II drugs require a DEA Form 222 to be sent to you prior to your sample shipment.  Please fill out the request below and return it, along with a copy of your DEA license, via email or fax.  Once we receive the completed form and license, we will overnight a Form 222 to you.
